After clicking the the “Help Request Form” you will see the google form and you have to fill the form with some of your correct information and then submit it we will reach you ASAP.

  4. Your Profession If you are a student then enter your institution name
  5. Name and address of the hospital where you are currently admitted
  6. Your medical expenses
  7. Photo of your hospital bill
